HQ Campus Track and Trace Form

A new eform has been introduced for recording visitors to the HQ Campus (comprising the HQ and Annexe Buildings), and for MC Staff who may have misplaced or lost their Staff ID Badge.

This information is required to give us an accurate account of personnel on site at any given time, so that this information can be passed to the National Track and Trace services if required.

The form can be used for three user types - 

Scheduled visitors -  you will be asked for contact details for your visitor, who will then be contacted and asked to complete a COVID declaration.  You will also receive an email to record when your visitor arrives on site, and another link to note when your visitor leaves also.

Visitors currently on Site - you will be required to provide contact details and record the COVID declaration on behalf of your visitor.  This form will record your visitor as having arrived, and you will be provided with  link to note when your visitor leaves.

Staff member lost/forgotten ID card - staff are asked to use this form on arrival if they do not have their Staff ID Badge - you will also receive an email to 'sign out' when you leave the campus.
